Dear Sir/Madam

Freedom of Information Request: A. Fire and Rescue Service Maternity leave policy, and, B. Fire and Rescue Service Trade Union leave policy.

I write to request the following information in respect of Name FRS.

I understand that the person responsible for dealing with freedom of information (FOI) requests for this fire and rescue service (FRS) can be contacted using the email address identified above.

If this is not correct, could I ask that either you pass this FOI request to the correct person for me, or, let me know that you are unable to assist me because

  1. the correct person is not contactable using the email address above, and
  2. you have no knowledge of how the correct person can be contacted.

The FRS may well have an online portal/ facility for making FOI requests. I do not wish to use such a portal/ facility.

I would like to request the following information:

  1. The current standard policy note that applies to uniformed (‘grey book’) employees concerning Maternity (which will include the issues such as reporting pregnancy; maternity leave; duration of maternity leave and maternity pay), and
  2. The current standard policy note that applies to uniformed (‘grey book’) employees concerning Trade Union leave (which will include issues such as how employees should apply for trade union leave, when trade union leave may be granted, perhaps definitions of trade union leave

I would like you to provide this information in the following format: Ideally the documents can be provided in Word format. If that is not possible – in PDF format.
